All incoming clients must meet with our clinical team for assessment. This is not a diagnostic assessment. The assessment will be to determine if placement at Centerpiece LLC is appropriate, obtain a history of services, what service level should be provided, and to gather the clinical information required to write the initial treatment plan if it is determined that placement in the program is appropriate.
The following items should be provided at or before the intake appointment:
- Copy of medical diagnosis
- Copy of most recent IEP
- Copy of insurance card and/or ForwardHealth card
- Any other pertinent records such as Birth to 3 assessments, psychological or neuropsychological evaluations, and records and/or discharge summary from past therapy provider. Patients who are bringing in information from a past therapy provider, please limit that information to treatment plans, assessments or discharge summaries. We do not need to see progress notes, daily therapy data, etc.
Families should plan to spend 1- 1.5 hours at Centerpiece LLC for the intake appointment. A tour will be provided during this time if the family has not already received one. Your child must be present for the intake appointment.
