FundingCenterpiece LLC accepts many types of insurance, Medicaid/ForwardHealth, county waiver funds, and private pay. Centerpiece LLC is happy to work with any companies that are not on the list. To request Centerpiece LLC becomes in in-network provider with a carrier not listed below, please call or email Centerpiece LLC. Centerpiece LLC is an in-network provider for the following insurance carriers:

Interested families should consult with their insurance carrier to verify if ABA therapy for autism is a covered benefit under the insurance plan. Patients are responsible for their deductible, coinsurance and copays (except when the patient has Medicaid coverage as a secondary insurance). Most insurance carriers require a prior authorization. This means that Centerpiece LLC must complete the intake process, draft a treatment plan, and request permission for the child to attend. After Centerpiece LLC receives the insurance approval, Centerpiece LLC will fit the child into the staffing schedule. This process can take several weeks, depending on the insurance carrier.

Centerpiece LLC provides services for the Medicaid/ForwardHealth behavioral treatment benefit at both the comprehensive and focused treatment levels. Families who wish to utilize this funding source must provide additional documentation as required by Medicaid. If you plan to use Medicaid, please be prepared to provide copies of your child’s IEP, diagnostic evaluations, and any other service records that your child may have (Birth to 3, etc). We will also need a prescription for therapy from the child’s pediatrician and the doctor will need to complete a health history questionnaire. The Centerpiece LLC staff will provide the forms that the pediatrician needs. This process can take several weeks, depending on how quickly the required documentation is provided.
